Parents, not programs, are the key to faith formation in children.

This assessment is designed to give you - and your church - insight into what will help you live into your role as the primary spiritual influence in your child's life. Discipleship begins at birth and the rhythms you establish now are laying essential groundwork for the next 18+ years.

We use the word DISCIPLESHIP a lot throughout this assessment. If that's an unfamiliar term to you, you can think of it this way: helping your kids to love and follow Jesus.

Please answer as honestly as you can. If you are stuck between multiple answers, don't overthink it! Go with your gut.

While your responses may be included in a summarized church report, they will always be anonymous. Your individual answers and results will not be shared with anyone but you.
